Uncovering the Hidden: A case of missed large Intra ocular foreign body.

Aim: To emphasize the significance of meticulous investigation and early treatment in ocular trauma. 
Intra-Ocular Foreign Bodies (IOFB) contribute 16-41% of open globe injuries, mostly in the posterior segment of the eye. Clinical presentation: A 19-year-old hosiery worker suffered an ocular injury while dashing off a cloth, causing immediate visual loss in the left eye. Primary corneal laceration repair was done elsewhere, after 2 weeks patient presented with pain, redness, watering and loss of vision. Vision was PL positive and PR inaccurate in the left eye and 6/9 in the right eye. B-scan and X-ray revealed a large IOFB with vitreous exudates and OTS was 39. He underwent micro incision vitrectomy surgery with IOFB removal (21mm iron nail) & silicon oil insertion. Conclusion: Patients history should not be solely relied upon. Comprehensive evaluation and early investigations are essential to minimize the incidence of endophthalmitis and vision loss caused by undetected IOFBs.
