Trauma

Traumatic anterior dislocation of lens: A rare but treatable cause of vision loss.

Purpose: To report a rare case of anterior dislocation of the lens. Materials and methods: We report a case of a 41-year-old male who presented with trauma to the right eye following a self-fall from the bike. He complained about swelling and sudden onset loss of vision in right eye. Visual acuity was perception of light, periorbital oedema and ecchymosis were noted. Intraocular pressure was 32 mmHg. Cataractous lens dislocated into the anterior chamber was noted. Results: Two tablets of acetazolamide 250 mg were given. Intravenous 20% mannitol 300 ml was given. He was posted for right-eye lenticular extraction with scleral fixated intraocular lens implantation under local anaesthesia. Vision improved to 6/12 at one month. Conclusion: Anterior ocular lens dislocation is rare, and blunt ocular trauma is one of the causes. At first, medical treatments, including cycloplegic and IOP-reducing drugs, should be started. Mainstay treatment is urgent surgical removal of the lens.
