Cornea and External Eye Disease
Corneal morphology, microbial flora, and exposure keratopathy in mechanically ventilated patients.
Purpose: To study the frequency of exposure keratopathy, microbes that dictate the ocular surface in mechanically ventilated patients at a tertiary care center for 1 year. Methods: Prospective cohort study, where 40 patients admitted to adult ICU were examined on days 3 and 7 after commencing ventilation. Assessment of corneal changes was done by fluorescein dye staining pattern with photographic documentation and a conjunctival swab taken for culture and sensitivity. Patients with keratopathy were treated and followed up. Results: Out of 40 patients,31 had corneal changes,26 showed growth with commensals,3 with S aureus, and 2 with pseudomonas resistant to local antibiotics, responding to colistin. Patients treated showed improvement in corneal changes. Conclusion: In chronic comatose patients, half-open eyes with infrequent blinking cause exposure keratopathy and antibiotic resistance. Proper eye care during ICU treatment helps prevent complications and thus preserve vision.
