Swimming through acute hydrops: compression sutures+ intracameral air injection combo to the rescue

ABSTRACT: 
AIM:Efficacy of combination of compression sutures,intracameral gas insertion in management of acute hydrops in keratoconus
Case presentation:A 20-year-old male presented with photophobia,sudden loss of vision in RE for 1 week.He was misdiagnosed elsewhere as viral keratitis and was on acyclovir.VA was cf 1/2meter right eye,6/24 in left eye.BCVA in RE no improvement,LE 6/9p.RE showed mild conjunctival congestion, central corneal edema.LE had Fleischers ring,thinning of cornea,Pentacam confirmed keratoconus.Topical steroids,hypertonic saline were started with no improvement.compression corneal sutures,intracameral air injection was done with vision 6/24 at 1 week.
Conclusion:Full thickness compression sutures,intracameral air helps in reducing corneal edema,early visual rehabilitation in acute hydrops.With the conservative approach,it takes a longer time to resolve with vascularization and scarring.This is a simple technique, with no need of special gases like C3F8 or SF6.
