WHEN THE VEINS BETRAY: CASE OF INFLAMMATORY CRVO ASSOCIATED WITH POSITIVE ANTICARDIOLIPIN ANTIBODIES

PURPOSE
To report a case of inflammatory CRVO with positive anticardiolipin antibodies
METHOD
28yr old male presented with sudden painless blurring of vision in right eye after awakening from sleep
No H/O trauma, similar complaint in fellow eye
Comprehensive Ocular, systemic examination was done
RESULTS
On examination right eye BCVA 6/24, anterior segment, color vision, IOP was normal.Fundoscopy revealed blurred disc margins with disc hemorrhages, dilated & tortuous veins with A:V 2:4, soft exudate adjacent to disc
Left eye vision, anterior, posterior segment was normal
Investigation revealed neutrophil 84%, CRP 7.10, positive anticardiolipin IgM (1.83)
Provisional diagnosis of inflammatory CRVO made
Patient started on Tab Prednisolone 50mg taper; Tab Aspirin 150mg OD with close monitoring
After 2months VA improved to 6/6(P), disc edema & hemorrhages resolved
CONCLUSION
It is important to find etiology at earliest to initiate prompt treatment which can lead to better visual prognosis
