Through the Veins, to the Eyes: Exploring the Ocular Effects of Dialysis and steroid therapy on IOP

Aim: To determine the association between haemodialysis, corticosteroid therapy, and new-onset glaucoma. 
Introduction: End-stage renal disease (ESRD), is a chronic condition; patients rely on maintenance dialysis or a renal transplant. Intra-ocular pressure changes during haemodialysis(HD) have been discussed in several studies. Clinical presentation: A 21yr old student with ESRD was started on HD and IV Methylprednisolone, which led to blurring of vision, an acute rise in IOP of >40mm of HG and optic disc changes. Despite various treatments, including oral steroids, maximum medical therapy with anti-glaucoma medications and renal transplant the IOP remained high. Finally, bilateral trabeculectomy was performed, resulting in reduced IOP and improved vision. Conclusion: Regular ophthalmic examinations in patients undergoing dialysis and corticosteroid therapy to identify individuals at risk of glaucoma and prevent permanent vision loss caused by elevated IOP
