OCCULOMOTOR NERVE PALSY CAUSED BY COMPRESSION FROM POSTERIOR CEREBRAL ARTERY

AIM: To report a rare case of LE oculomotor nerve palsy caused by compression from Left posterior cerebral artery.
DISCUSSION:A 28 year old male patient presented with complains of drooping of LE upper lid and lateral deviation of the LE since 7years.Vision in the LE is 6/12.On ophthalmic examination moderate Ptosis noted in the LE and restriction of ocular movements noted for abduction ,elevation, depression, levoelevation, levodepression. 5mm fixed dilated pupil noted in the LE .Poor Bell’s phenomenon noticed. MRI showed lateral deviation of left globe, atrophic left medial rectus and indentation of cisternal part of left oculomotor nerve by left posterior cerebral artery.
CONCLUSION: The classical description of PUPIL INVOLVED oculomotor nerve palsy serves to aid the clinician in diagnosing a surgical cause of oculomotor nerve palsy. Aberrant vasculature of posterior cerebral artery is a rare but possible cause of oculomotor nerve palsy.
