“OCULAR DANGER IN DEVIC’S DISEASE”

Abstract-Neuromyelitis optica spectrum disorder is an inflammatory demyelinating disorder of the central nervous system  involving the optic nerve and spinal cord ,characterised by recurrent/bilateral attacks of optic neuritis 
Case report: 32 year old female presented with diminution of vision in both eyes with painful eye movements and headache.Vision in right eye was counting finger close to face and in left eye no perception of light with bilateral afferent pupillary defect. Fundus examination revealed bilateral pale optic disc.Anti-Aquaporin 4(NMO) IgG antibodies- strongly positive
Conclusion:Severe or recurrent bilateral vision loss with posterior predominant extensive optic nerve involvement are important clinical pointers towards NMOSD. Given the prognosis and the potential of the disease to recur, early diagnosis and prompt management with aggressive immune-modulation is required. Prompt diagnosis is important as full recovery of vision is possible if treated on time.
