“A RARE PRESENTATION OF CARCINOMA BREAST MANIFESTING WITH LATERAL RECTUS PALSY”

ABSTRACT
Sixth nerve palsy is the most common isolated cranial nerve palsy.It innervates the lateral rectus and is responsible for ipsilateral eye abduction.
Case report: 
A 44 year old female with Ca breast stage IV on treatment  presented with complaints of headache,diplopia since 2 weeks and diminution of vision since 1 day.
On ocular examination,VA was 6/24 OD, PL+ve PR inaccurate OS with restriction of abduction in both eyes with horizontal binocular diplopia and 30 degree esotropia in the left eye.
MRI brain showed arachnoid cyst measuring 21x21mm in the posterior fossa ,compressing right cerebellar hemisphere,no surrounding edema.

Conclusion:
We conclude that appropriate eye examination can be a feasible method to clinch a diagnosis of intracranial spread of certain malignant conditions in the era of sophisticated investigations and thus help in adequate and timely management of such cases.
