 A case of suprachoroidal hemorrhage with choroidal detachment post systemic thrombolytic therapy.

Purpose:To report a case of suprachoroidal hemorrhage with choroidal detachment post systemic thrombolytic therapy
Method:A 48 year old male presented with pain,DOV and redness in LE.He had received inj.streptokinase for MI 5 days before.On examining LE V/A was PL+ associated with corneal edema,neovascularisation of iris,shallow anterior chamber and fixed dilated non reactive pupil with IOP 36mm of hg.B scan showed suprachoroidal hemorrhage with choroidal detachment.RE was within normal limits.
Results:1 dose of 100ml IV mannitol was given and IOP was maintained in the range of 20-22mm hg with tab.acetazolamide BD and e/d dorzolamide BD.He was put on topical antibiotics and steroids with oral steroids.V/A on the 3rd day of presentation improved to HM+. 
Conclusion:Intra-ocular hemorrhage is a rare adverse effect of streptokinase.Early identification of ocular alarming signs and control of IOP may prevent irreversible damage to the optic nerve and significantly improve the prognosis.
