A Rare case of Idiopathic orbital inflammatory syndrome with late response to steroid treatment. 

A 54-year-old man presented with swelling and pain of the right eye that was associated with proptosis, ptosis and restriction of eye movements in all gazes which was progressive since 2 months. A differential diagnosis of orbital cellulitis, thyroid orbitopathy, orbital neoplasm, lymphoproliferative disorder and idiopathic orbital inflammatory syndrome (IOIS) was considered. Relevant laboratory investigations were performed, which were normal. Magnetic resonance imaging was suggestive of IOIS. The patient was started on IV Steroids (Methyl Prednisolone) with IV antibiotics, but did not show clinical improvement. Patient was discharged with oral steroids. Patient was called for follow up after 1 week. Patient started showing improvement 1 week after discharge. Patient was followed up regularly. After 4 weeks of oral treatment patient showed symptomatic improvement.
