HOLE IN THE HILL- PERIPUNCTAL MELANOCYTIC NAEVUS

PURPOSE – To report a case of peripunctal melanocytic naevus
METHOD – Peripunctal naevi are rare lesions of eyelid, accounts for half of benign peripunctal tumors. 43-year-old female had complaints of itching & left lower lid medial lesion since birth without associated epiphora. There was no change in size/color since a decade. On examination, pigmented, dome shaped lesion was noted in the  at the junction of pars ciliaris & pars lacrimalis. Brownish black lesion  circumferential to punctum with opening on top of it with ramifying vascularity on the surface. The punctal opening was slit shaped. Lacrimal syringing was patent & without resistance on probing.
RESULTS- Surrounding lid skin, margins, conjunctiva were free of any infiltration. Diagnosis of left lower lid peripunctal naevus was made & options of shave excision biopsy/observation were discussed.
CONCLUSION- Patient refused treatment as she was asymptomatic. She was advised for regular observation.
