A case study on refractive status and morphology of bilateral anterior lenticonus in Alport Syndrome

Aim: To evaluate by AR and A scan with AS OCT imaging
Case history: A 33 year old male patient undergoing hemodialysis since 3 years presented with bilateral diminished vision since 7 years. Uncorrected distant visual acuity was 6/18 and 6/24, near vision N12 and N18 with no pinhole improvement. A circumscribed dark shadow in the central cornea on DDO noted and correlated with oil droplet configuration on retroillumination.  Myopic retinoscope reflexes observed and autokeratorefractometer measured over-spheres with 2D of astigmatism but with poor acceptance. A-scan measured AL of 23.61 and 22.86 mm, and LT of 3.95 and 3.98 mm respectively. A conspicuous well demarcated lenticonus image captured on 512x128 cube on AS OCT. Bilateral sensorineural hearing loss was detected on audiometry.
Conclusion: Although an unacceptable high myopia possibly due to lenticular origin was detected, however not quantifiable. Improved imaging analysis was possible to study the distinct features on AS OCT.
