Mysterious blue eye:Case of Acute hydrops due to longterm steroid use in vernal keratoconjunctivitis

Purpose:Reporting case of acute corneal hydrops in vernal keratoconjunctivitis(VKC)
Method:8yr male came with C/O sudden dimunition of vision, pain and bluish discoloration of left eye(LE)for 15 days.He was on topical steroids for 6 months for VKC.The vision in the LE was hand movements.Slit lamp findings were conjunctival congestion(CC),giant papillae(GP)were seen,Cornea was edematous with epithelial bullae and pressures raised in LE.Posterior segment findings with B-scan was normal.Right eye showed CC,GP with early posterior polar cataract.Immediate cessation of steroids alternated with immunosuppressants, antiallergics was advised combined with anti-glaucoma drugs, cycloplegics
Result:Resolution of Corneal edema with stromal scarring and better vision was seen on 1month follow-up
Conclusion:This highlights possibility of steroid-induced glaucoma in VKC due to indiscreet steroids use, timely intervention to prevent irreversible blindness due to optic nerve damage and corneal scarring.
