Smoldering Vogt-Koyanagi-Harada disease with extra ocular involvement  

Aim- To report a case of smoldering Vogt-Koyanagi-Harada disease with extra-ocular involvement
Methods and materials: A 45year old male presented with DOV in BE since 1week with redness,pain,watering, light intolerance with tinnitus. O/E-BE visual acuity CF1/2 m. Slit lamp ex. showed conjunctival ingestion, mutton fat KP’s, & sluggishly reactive pupil with multiple broken posterior synechiae. Fundus showed grade 2 vitreous cells with multiple opacities, diffuse retinochoroidal thickening & disc edema. Tonometry-16 and 29mmhg. B-scan showed multiple vitreous echoes, diffuse retinalchoroidal complex thickening with multiple shallow serous RD. On routine investigations, patient was started on topical and systemic steroids. LP - 82% lymphocytic pleocytosis. Patient recovered well with 6/24 vision in BE with resolution of fundus finding at 6-week F/U.
Conclusion - high index of suspicion ,timely diagnosis and management is crucial to prevent vision threatening complications in VKH disease.
