Bilateral optic disc coloboma with gyrate atrophy

Aim: To detail the findings in a rare case of bilateral optic disc coloboma with gyrate atrophy.
Methodology: A 68 year old male patient presented with a history of painless, gradually progressive diminution of vision, more at night since childhood. His vision was OD-6/24 and OS-counting fingers at 1 m. Detailed ophthalmic evaluation including slit lamp examination, dilated fundoscopy done. He underwent cataract surgery in left eye followed by neurological examination and radiological investigations. 
Result: Visual acuity in left eye post cataract surgery was 6/24. He had bilateral decreased visual acuity with pseudophakia and bilateral symmetric optic disc coloboma with characteristic chorioretinal atrophy suggestive of gyrate atrophy. Visual fields were tubular in both eyes. MRI brain - only diffuse cerebral atrophy. 
Conclusion: This case report is a presentation of rare co-existence of bilateral optic disc coloboma with gyrate atrophy that necessitates detailed evaluation.
