SAFETY AND EFFICACY OF SODIUM BICARBONATE VERSUS HYALURONIDASE IN PERIBULBAR ANAESTHESIA 

Purpose:To compare the efficacy and safety of Peribulbar anesthesia with Sodium bicarbonate and Hyaluronidase for cataract surgery in terms of globe akinesia, analgesia,onset and duration of anesthesia and complications 
Method:182 patients in this prospective,double blind, randomised study grouped into A & B of 91 each, received peribulbar injection of 2% Lidocaine with Hyaluronidase and 2% Lidocaine with Sodium Bicarbonate under aseptic precautions.They were compared for effectiveness of the anaesthesia, its onset, duration and any complications. 
Results:Sodium bicarbonate with lidocaine had better analgesia (75%) and akinesia (45%) than hyaluronidase (64.1%&43.6%). No difference in early onset of anesthesia <10min as noted in group A(92.3%) and group B(92.5%). Similarly, subconjunctival hemorrhage and chemosis was less in group B (10.3%, 23.1%) than group A (12.5% & 27.5%) 
Conclusion:Sodium bicarbonate clinically achieves a better analgesia, akinesia with minimal adverse effects.
