A CASE OF POSTERIOR SEGMENT INTRA-OCULAR FOREIGN BODY

PURPOSE- A case a metallic intra ocular foreign body in the posterior segment.
METHODS- A male mechanic, with history of trauma to the right eye, came with complaints of sudden decrease in vision and ocular pain. On examination vision was PL +, PR inaccurate.  Anterior segment showed hazy cornea,hyphema and full thickness corneal tear with  iris incarceration. B scan showed homogenous hyper-reflective echoes suggestive of vitreous hemorrhage. CT orbit showed a radio opaque  foreign body 8.5mm x 6.7mm in the  posterior segment. Primary repair and globe exploration was done followed by Pars plana vitrectomy with IOFB removal with silicone oil implantation. Patient was left aphakic and secondary IOL implantation was done after 1 month. 
RESULT- Patient had improved vision to 6/9p with no post-operative complications.
CONCLUSION- Meticulous primary repair and subsequent surgeries have a better visual prognosis in trauma patients.
