Diabetic Retinopathy & Medical retina
A Rare case of Endogenous Serratia marcescens endophthalmitis leading to complete visual loss.
59-year-old diabetic male presented with acute onset left ocular pain, redness & blurred vision. On examination left eye visual acuity was perception of light; with inaccurate PR. Anterior segment examination showed left eye upper eyelid edema, circumcorneal congestion, 2.5 mm hypopyon, mid-dilated pupil & IOP 25 mmHg. Right eye examination- WNL. Left eye B-scan showed vitreous echoes & RCS complex thickening. Patient was diagnosed with Endogenous endophthalmitis and intravitreal Voriconazole, Ceftazidime and Vancomycin were given and AC & vitreous tap were sent for culture. Culture was reported positive for Serratia Marcesens. His blood and urine cultures were negative. Despite which the following day there was corneal perforation with extrusion of lens, uveal tissue and vitreous prolapse and patient underwent Evisceration. This case highlights the virulence, invasive nature of Serratia Marcesens causing endophthalmitis which required evisceration and lead to complete visual loss.
